It was once a hush-hush business that even Hollywood stars were at pains to hide but plastic surgery has come of age, another  lifestyle choice in a world full of possibilities.

Despite few official figures being collated in Australia, plastic surgeons report a growing desire for cosmetic plastic surgery, particularly in young women, some yet to finish high school.

Following in the footsteps of Americans, the Australian trend towards early plastic surgery prompted NSW Premier Morris Iemma last month to call for compulsory counselling and a ban on all procedures for patients under 16.

His suggestion was met with a range of responses and far from dismissing such moves, the Australian Society of Plastic Surgeons, while it stopped short of supporting a ban, backed the call for reform and urged caution where teenagers were concerned.

At the heart of the debate is the age-old question of whether we should grow old  gracefully or is the odd nip and tuck, even for our young ones, justified?

Chasing the body beautiful is not a new desire  people have always wished for smaller ears, a button nose or bigger breasts and people have been exploring ways to drink from the fountain of youth for centuries.

What has changed, says plastic surgeon Patrick Briggs, is that people feel plastic surgery is more accessible.

 There is more of an awareness and an understanding of cosmetic surgery and people have realised that you don t have to be rich or an heiress like Paris Hilton to have it, he said.

 The whole issue is basically about self-esteem, whether plastic surgery is performed on people who have congenital defects, have suffered trauma or have a growth abnormality, it s all about looking and feeling better about themselves.

Dr Briggs said when it came to younger patients, each request for cosmetic work was assessed on an individual basis.

 What we like to focus on is motivation  we need to be very careful about the reasons for young patients wanting to have surgery. The key to a successful procedure lies in making sure people are not under excess pressure from anyone else, say a 17-year-old boyfriend who wants his girlfriend to have bigger breasts, for example.

 If a 16 or 17-year-old girl comes in wanting a breast augmentation (enlargement), then the first question we ll ask is about her menarche (first menstrual period). If it was only last year, then the chances are that she is still growing breast tissue and an augmentation is not appropriate at that time.

In some cases, there was parental pressure for a procedure to be performed and a staged decisionmaking process, along with two or more consultations, allowed people to weigh up their decision carefully, he said. In other cases where growth was an issue, procedures were performed at much earlier ages.

 We often do bat ear surgery any time over the age of seven because ears stop growing then and it s something the Health Department will fund, Dr Briggs said.  If a child of 10 or 12 is being unmercifully teased and is failing in their school work, then that type of procedure can really turn their life around. While such a degree of emotional improvement is rare, most people feel better after surgery, regardless of what they are improving.

Women were still the biggest consumers of cosmetic surgery and were well versed and educated on what they desired. Men, on the other hand, had less understanding of the process.

 When a woman comes to me, she knows an awful lot  she has done a lot of detailed research from books and on the internet and has normally given it quite a lot of thought, Dr Briggs said.

 When a man comes in, all he knows is that he wants to change something about himself.

Men usually wanted to change their noses and reduce male breasts, while women requested everything from breast enlargement, to liposuction, laser therapy, eye, face and brow lifts and botox injections.

Young women were more likely to want a breast augmentation or reduction or rhinoplasty (nose job).

Breast enlargement usually cost between $11,000 and $12,000 including anaesthetic and hospital fees and the cost of implants, while a reduction cost around $9000. Nose jobs usually cost between $8000 and $9000 and liposuction was around $2000 for each distinct area of the body such as legs, stomach and buttocks.
